To apply for a new account you must complete and fax the following forms to (304) 725-9413:

1. New Account Form
    
2. Tax Exempt Form for your state (Virginia, Maryland, West Virginia)
    
3. Taxpayer Identification Number and Certification Form
    
4. If applying for credit - Credit Application, Credit Release Form, and Credit Reference Form
