To apply for a new account you must complete and fax the following forms to (304) 725-9413:
- New Account Form
- Tax Exempt Form for your state (Virginia, Maryland, West Virginia)
- Taxpayer Identification Number and Certification Form
- If applying for credit - Credit Application, Credit Release Form, and Credit Reference Form
